Regional Medical Officer – Western Region

What attracted me to InnovAge was the thought of serving seniors and their families in such a way that allows them the support they need to remain in their homes. Together, we can ensure their golden years are enjoyed to the fullest.

A native to California, Dr. Jauregui has been the regional medical director for SupportiveCare Medical Group, which serves several Southern California counties. His medical experience includes serving as the chief study investigator for multiple palliative care programs through UniHealth Foundation grants.
 
He received his medical degree from the School of Medicine at the University of California, San Diego, and is board-certified in family medicine, palliative medicine, and as a physician executive with the American College of Physician Executives.

Taking care of people and listening to their stories gives me immense satisfaction and I am happiest when I can help someone along their healthcare path. I feel privileged to be a part of the PACE team at InnovAge and help our seniors live the life they deserve.

Jeanette Williams joined the InnovAge team in 2019. She has more than 20 years of experience in healthcare leadership. Her experience encompasses running long-term acute care, acute rehab hospitals, home healthcare, and skilled nursing. She holds a master’s degree in business administration from University of Phoenix.

Center Director

Working with seniors is both an honor and a privilege. The opportunity to have such a positive impact on our participants’ overall quality of life, while building relationships that facilitate learning from our population’s wealth of life knowledge, is a true blessing!

Steve Jones was named center director of the Inland Empire PACE center in 2019. He has been with InnovAge since 2014, first as a rehabilitation aide and then the clinical exercise services manager at the Denver PACE center. While at the Denver center, Steve launched the clinical exercise services (CES) program, which has since been expanded to other InnovAge centers. He has a degree in exercise physiology and is currently pursuing a master of business administration degree in healthcare administration from Colorado Christian University.

Dr. Siraj is board certified in family and geriatric medicine. She earned her doctor of
medicine from Kempegowda Institute of Medical Science in India. Dr. Siraj completed her residency at the University of Arkansas for Medical Science and her fellowship at the Reynolds Institute on Aging. Prior to joining InnovAge, Dr. Siraj worked in private practice, served as a mentor to medical students and residents and was a medical
director for hospice.

Dr. Siraj is married and has two children. She enjoys swimming, reading and traveling.

Nurse Practitioner

At PACE we care for the whole person. This is done through collaboration, compassion and genuinely listening to participants.
 

Charlisa earned her Bachelor and Master of Science Degrees in Nursing from Chamberlain University in Illinois. She is board certified in family practice through the American Academy of Nurse Practitioners. She is a member of the American Academy of Nurse Practitioners and the California Association for Nurse Practitioners.
She has experience working as a traveling registered nurse in many settings including; critical care, emergency care, geriatrics, cardiology and neurology.
Charlisa grew up in Kentucky and lived in many states as a traveling nurse. She is the youngest of ten children. Charlisa is married and has a spoiled Yorkie. She enjoys reading, watching movie and sports, and exploring new places.

Nurse Practitioner

I want to provide PACE participants with the best care. The same care I want to have when I am their age.

Crystal earned her nursing degrees at Azusa Pacific University. She is board certified as a family nurse practitioner by the American Association of Nurse Practitioners. While she was in college, she worked as a volunteer at the Veterans Administration (VA) Loma Linda Healthcare System in the emergency department and intensive care unit.

Crystal is a home tutor and has been teaching Mandarin for more than 10 years. She also speaks Chinese. Crystal lives in Redlands, CA and has a teenage son. She enjoys hiking, jogging and spending time with family.

InnovAgePACE is very important for seniors. I enjoy providing them with complete dental care they may not have had access to in the past.

Dr. Huigens earned his Doctor of Dental Surgery Degree (DDS) from the University of Oklahoma Health Sciences Center, College of Dentistry where he graduated with honors. He also earned a Physician Associate Degree from the University of Oklahoma College of Medicine and a bachelor degree in biology from the University of LaVerne in CA.

Upon graduation, we was in private dental practice in La Verne, CA. Dr. Huigens has a special interests in dental implants and dental appliances to treat sleep apnea. He also volunteered as a dentist at a low-income children’s dental clinic.

Dr. Huigens has always been interested in healthcare and nearly every job he’s held has been in the medical profession. As a college student, he worked as a paramedic and as a physician associate at an orthopedic surgery practice.

In his spare time, Dr. Huigens enjoys art, music, do-it-yourself projects, working on his car and spending time with his son.

Primary Care Doctor

I believe in PACE and its multidisciplinary team approach to geriatric care. This let’s our participants focus on what is important in their lives.
 

Dr. Tang earned his medical degree from the A.T. Still University School of Osteopathic Medicine in Arizona. He also completed a family medicine residency at CarePoint Christ Hospital in New Jersey and a geriatric medicine fellowship at the University of California in Irvine, California. Dr. Tang is board certified in Family Medicine.


Dr. Tang is fluent in Mandarin. He enjoys traveling, biking, hiking and exploring new areas.

Nurse Practitioner

Older adults want to remain as independent as possible and live in their own homes. It is important to me to help them live life as they wish.
 

Rani received her Bachelor of Science Degree in Nursing from Grand Canyon University and a Master of Science Degree in Nursing from Walden University. Prior to joining InnovAge, she worked as a nurse practitioner in a primary care clinic and as a registered nurse and charge nurse in an intensive care unit.
 
She speaks Malayalam and Hindi. Rani enjoys spending time with family and friends and volunteering at her church. She likes to take walks with her Golden Retriever, Max.

Reni earned her Bachelor of Science Degree in Nursing at Bangalore University in India and her Master of Science Degree in Nursing from Azusa Pacific University. She is board certified family nurse practitioner by the American Association of Nurse Practitioners. Reni is also a member of the California Association of Nurse Practitioners.
 
She worked as a nurse in India and Qatar. Reni moved to California and worked at Saint Bernardine Medical Center in the telemetry and direct observation units. 
 
Reni is married and has two children. She enjoys listening to music, travelling and spending time with family. She speaks Indian languages such as Hindi, Kannada, Tamil, and Malayalam.

Faith earned her Bachelor’s and Master’s Degrees in Nursing from Western Governors University. She earned a post-master’s degree certificate in Family Nurse Practice from Purdue Global University. Faith is board certified with the American Association of Nurse Practitioners and a California State Certified Family Nurse Practitioner.  She has nursing experience in a variety of family care settings including hospice care.

She is married, has three children and seven grandchildren. Faith enjoys Christian music and traveling.
