Center Director

From an early age, I knew I had a passion for helping those in need. My passion quickly grew towards the elder adult population and continues to grow daily. Giving back to those who have paved the way for myself and others is rewarding, and being part of InnovAge allows me to do so every day.

Chad Perkey joined InnovAge in 2018 as center director of the Aurora PACE center. He is a Licensed Nursing Home Administrator (LNHA) and has more than 11 years in various leadership positions with senior living facilities in Colorado and Virginia, including Alpine Living Facility and the Villas at Sunny Acres. Chad earned his master’s degree in health promotion and education from Virginia Tech University, as well as his bachelor’s degree from Radford University. In 2012, he received the Martin Luther King Jr. Certificate of Recognition for “Uniting Our Voices for Change” from the Fauquier County Chamber of Commerce in Virginia.

Regional Medical Officer – Central and Western Regions

My father always said, "It takes courage to age." I enjoy working with the interdisciplinary team at InnovAge, and courageous seniors, to keep our worlds large, and our time meaningful.

Dr. Strange earned her Doctor of Medicine (MD) from the St. Louis University School of Medicine, and is a diplomate with the American Board of Family Practice. A Colorado native, she has spent her career in private practice and residency faculty positions, teaching the next generation of family physicians in Pueblo. In 2015, the Association of Family Medicine Residency Directors gave her the Bronze Program Director Award. Under her leadership, Southern Colorado Family Medicine in 2012 and 2015 received NCQA Level 3 recognition for providing patient-centered medical care. Dr. Strange’s husband is also a family practitioner and they have three children. She enjoys volunteering with her church, riding bicycles and hiking.

Medical Director

Aging and illness sometimes make it hard to experience quality living. My goal is to help ease those difficulties so patients can enjoy life and participate in things that make them happy.

Dr. Litvak enjoys coming to work to learn from others, including patients, and is proud to be part of a medical team devoted to helping older adults. Following in her father’s footsteps, Dr. Litvak pursued a career as a physician. She earned her Doctor of Medicine (MD) from the University of Colorado School of Medicine. She has worked in hospitals and private practice, including St. Anthony Hospital Systems and their affiliated clinics. She is board certified in Family Medicine. Dr. Litvak grew up in Michigan and has lived in Colorado since 1968. She is happily married and has three sons. She enjoys many hobbies such as softball, gardening, bonsai, travel, reading and golf.

Dr. Kimball is a board certified wound management specialist physician with additional qualifications in undersea and hyperbaric medicine, vascular medicine, geriatrics and surgery. She has a passionate focus on being a key opinion leader in the wound care industry and introducing innovative human designs that seek to improve the quality and delivery of care to patients.
Dr. Kimball received her medical degree from the University of Texas Medical School, completed her general surgery residency at the University of Colorado Anschutz Medical Campus, and completed vascular surgery research at the University of Utah. Dr. Kimball's primary professional goal is to build a thriving, self-sustainable wound management practice.
Dr. Kimball has two children, a son and a daughter.

Dr. Hager’s favorite thing about working with aging adults is the lifetime of unique experiences they bring with them, and their “admirable resilience.” He considers it a professional success when he can “genuinely help change lives in some significant way,” and appreciates the “wonderful staff” at the PACE Center.
Dr. Hager attended The George Washington University School of Medicine in Washington, D.C. where he received his Doctor of Medicine (MD) and a Master of Public Health (MPH). He completed his residency in Internal Medicine in 2002 and is board certified in Internal Medicine.
Dr. Hager has lived in California, Pennsylvania, and France, where he spent a semester at the University of Paris-Sorbonne. He has two children and is an avid long distance runner who also enjoys photography, writing and meditation. Dr. Hager is bilingual in English and French.

Improving the quality of life for my patients is what drew me to geriatrics and what I strive to provide at InnovAge.

Dr. Libbon is board certified in internal medicine. He earned his medical degree from The University of Arizona in Tucson. He completed his residency in internal medicine and a fellowship in geriatric medicine at the University of Colorado.

Dr. Libbon is a volunteer faculty member at The University of Colorado and conducts research.

Dr. Libbon is married, has one child and an energetic dog. He enjoys swimming, photography, filmmaking and music composition, arrangement and performance. He likes to watch the Golden Girls.

My goal is to be compassionate ad attentive to all of the patients in my care.

Dr. Rivera is board certified in internal medicine. She earned her medical degree from the University of Santo Tomas in Manila, Philippines and completed her residency at Jewish Hospital of Cincinnati.

Dr. Rivera strives to foster a sense of family and community at the PACE center. Prior to joining InnovAge, Dr. Rivera worked at University of Cincinnati Health. She moved to Colorado with her husband in 2015. She has over 20 years of clinical experience in caring for patients.

Dr. Rivera is married and has three children. She loves to cook, travel, garden, read, spend time outdoors and attend Zumba classes.

Primary Care Doctor

I try to make a personal contact with every patient. At times, it is more important to talk about a mutual interest or a story from the past than it is to fuss over clinical details.

Dr. Ryndfleisz’s training is unusual. He spent half his residency in a rural town working with a family practitioner, a general surgeon, and an orthopedic surgeon. He served as Chief of Medical Staff and Medical Director of a skilled nursing facility. He enjoys working with the Interdisciplinary Team at PACE and working with aging adults due to their wisdom and their often challenging medical needs.
He received his Doctor of Osteopathic Medicine (DO) from the Kirksville College of Osteopathic Medicine in Missouri. He completed his residency in 1994 and is board certified in Family Practice by the American Osteopathic Board of Family Physicians. He specializes in Family Medicine and Geriatrics. He considers his help with a state trauma system in Colorado to be his greatest professional achievement.
He is married with a teenage daughter, son, and many pets. He enjoys fishing, astronomy, music, and American history.

Nurse Practitioner

As an InnovAge provider, I am motivated to provide quality, preventative healthcare and serve as an advocate for our participants.

Teresa Tanabe is board certified in adult gerontology primary care. She earned her medical degree from University of California, Los Angeles. Prior to joining InnovAge, Teresa worked as a medical-surgical nurse, a geriatric nurse and a nurse practitioner in a variety of medical centers. Her passion is working with the senior population.

Teresa was born and raised in southern California and moved to Colorado in 2015. She is married and has several pets including cats, fish, chickens and a dog. She enjoys cooking, hiking and spending time with loved ones.
