Regional Medical Officer – Central Region

My father always said, "It takes courage to age." I enjoy working with the interdisciplinary team at InnovAge, and courageous seniors, to keep our worlds large, and our time meaningful.

Dr. Strange earned her Doctor of Medicine (MD) from the St. Louis University School of Medicine, and is a diplomate with the American Board of Family Practice. A Colorado native, she has spent her career in private practice and residency faculty positions, teaching the next generation of family physicians in Pueblo. In 2015, the Association of Family Medicine Residency Directors gave her the Bronze Program Director Award. Under her leadership, Southern Colorado Family Medicine in 2012 and 2015 received NCQA Level 3 recognition for providing patient-centered medical care. Dr. Strange’s husband is also a family practitioner and they have three children. She enjoys volunteering with her church, riding bicycles and hiking.

From an early age, I knew I had a passion for helping those in need. My passion quickly grew towards the elder adult population and continues to grow daily. Giving back to those who have paved the way for myself and others is rewarding, and being part of InnovAge allows me to do so every day.

Chad Perkey joined InnovAge in 2018 as center director of the Aurora PACE center. He is a Licensed Nursing Home Administrator (LNHA) and has more than 11 years in various leadership positions with senior living facilities in Colorado and Virginia, including Alpine Living Facility and the Villas at Sunny Acres. Chad earned his master’s degree in health promotion and education from Virginia Tech University, as well as his bachelor’s degree from Radford University. In 2012, he received the Martin Luther King Jr. Certificate of Recognition for “Uniting Our Voices for Change” from the Fauquier County Chamber of Commerce in Virginia.

Medical Director

Every person has a story and unique experiences in life. Seniors deserve to be able to maintain their dignity and independence as much as possible, which is of the utmost importance to me.

Dr. Richman earned his Doctor of Medicine (MD) degree from the University of Buffalo, School of Medicine. He is board certified in physical medicine and rehabilitation. Teaching future physicians has been important to him throughout his career. In addition to leading PACE physicians across all InnovAge Colorado PACE centers, Dr. Richman is also an associate clinical professor at the University of Colorado Medical School. He has worked in leadership roles in private practice and hospitals. He has a special interest in working with stroke and brain injury patients.

Dr. Richman is married, has two sons, one cat and one dog. He has a varied list of interests and hobbies that includes dogs, football and music. He and his wife are recognized breeders of rare Olde English Bulldogges. As a Buffalo, NY native, he is a big fan of the Buffalo Bills. He also played the French horn with the Buffalo Philharmonic.

Medical Director

Aging and illness sometimes make it hard to experience quality living. My goal is to help ease those difficulties so patients can enjoy life and participate in things that make them happy.

Dr. Litvak enjoys coming to work to learn from others, including patients, and is proud to be part of a medical team devoted to helping older adults. Following in her father’s footsteps, Dr. Litvak pursued a career as a physician. She earned her Doctor of Medicine (MD) from the University of Colorado School of Medicine. She has worked in hospitals and private practice, including St. Anthony Hospital Systems and their affiliated clinics. She is board certified in Family Medicine. Dr. Litvak grew up in Michigan and has lived in Colorado since 1968. She is happily married and has three sons. She enjoys many hobbies such as softball, gardening, bonsai, travel, reading and golf.

It’s my privilege to care for older adults. I love learning about their experiences and passions and strive to focus on what is most important to them when providing care.
 

Dr. Rogers earned her medical degree from the University of Texas Medical Branch in Galveston, TX. She also completed an internship/residency in Internal Medicine at the University of Alabama and a Geriatric Medicine Fellowship at the University of Colorado. She is board certified in Internal Medicine and Geriatrics.

She has special interests in preventative care, women’s health, and helping reduce caregiver burden.

Dr. Rogers’ husband is a pediatrician and they have a son. Their home also includes two cats and two dogs. She grew up in Texas. She and her family enjoy the weather and the people in Colorado. Dr. Rogers likes to hike, read, walk her dogs, travel and sample new restaurants in Denver. She speaks some Spanish and is always looking to practice with others who are more fluent in the language.

Dr. Halligan received her Doctorate of Dental Surgery from the Baylor College of Dentistry at Texas A&M. She completed her residency in Hospital Dentistry at Memorial Hermann Hospital in Houston, TX. Her professional memberships include the Royal College of Surgeons of Edinburgh,Scotland in Special Needs Dentistry. Only thirteen dentists in the U.S. hold this membership. She is also a diplomat with the American Board of Special Care Dentistry. Throughout her career, Dr. Halligan has worked in private practice and teaching post and pre-doctoral dental students. Dr. Halligan is married to an elementary school music teacher and has three children. She and her family like to hike and bike. She has been inspired and has a desire to learn sign language.

Dr. Peng earned her medical degree from the New York University School of Medicine with a concentration in health policy. She also completed a residency in internal medicine and a fellowship in geriatrics at the University of Colorado. She is board certified in internal medicine.
 
She has a strong commitment to working with the underserved and educating future medical professionals. She is a volunteer faculty member at the University of Colorado and teaches geriatrics to medical students. Many of her students spend time with her at the InnovAge PACE center to further their education. In the past, she was a medical volunteer at free clinics in Colorado and New York.
 
Dr. Peng speaks Mandarin and was born in Taiwan. She moved to New York when she was 12 years old. She plays the violin and enjoys going to the symphony and ballet. Dr. Peng also likes hiking and travel.

Dr. Kimball is a board certified wound management specialist physician with additional qualifications in undersea and hyperbaric medicine, vascular medicine, geriatrics and surgery. She has a passionate focus on being a key opinion leader in the wound care industry and introducing innovative human designs that seek to improve the quality and delivery of care to patients.
 
Dr. Kimball received her medical degree from the University of Texas Medical School, completed her general surgery residency at the University of Colorado Anschutz Medical Campus, and completed vascular surgery research at the University of Utah. Dr. Kimball's primary professional goal is to build a thriving, self-sustainable wound management practice.
 
Dr. Kimball has two children, a son and a daughter.

Improving the quality of life for my patients is what drew me to geriatrics and what I strive to provide at InnovAge.

Dr. Libbon is board certified in internal medicine. He earned his medical degree from The University of Arizona in Tucson. He completed his residency in internal medicine and a fellowship in geriatric medicine at the University of Colorado. Dr. Libbon is a volunteer faculty member at The University of Colorado and conducts research. Dr. Libbon is married, has one child and an energetic dog. He enjoys swimming, photography, filmmaking and music composition, arrangement and performance. He likes to watch the Golden Girls.

I’m motivated at work because I know that every participant can teach me as much about humanity as I can teach them about staying healthy.

Dr. Poudyal is board certified in internal medicine and nephrology. She earned her medical degree from University of Texas Southwestern Medical School and completed her residency at Yale-New Haven Hospital in Connecticut.

Prior to joining InnovAge, Dr. Poudyal was a nephrologist. She strives to make high quality medical care accessible to everyone. She volunteers in primary care clinics to serve those that are uninsured and is passionate about developing programs and scholarship opportunities for children and young women in developing communities.

Dr. Poudyal grew up in West Texas and moved to Colorado in 2006. In her free time, Dr. Poudyal enjoys jogging, listening to 80’s music and reading.

My goal is to be compassionate ad attentive to all of the patients in my care.

Dr. Rivera is board certified in internal medicine. She earned her medical degree from the University of Santo Tomas in Manila, Philippines and completed her residency at Jewish Hospital of Cincinnati.

Dr. Rivera strives to foster a sense of family and community at the PACE center. Prior to joining InnovAge, Dr. Rivera worked at University of Cincinnati Health. She moved to Colorado with her husband in 2015. She has over 20 years of clinical experience in caring for patients.

Dr. Rivera is married and has three children. She loves to cook, travel, garden, read, spend time outdoors and attend Zumba classes.

Nurse Practitioner

As an InnovAge provider, I am motivated to provide quality, preventative healthcare and serve as an advocate for our participants.

Teresa Tanabe is board certified in adult gerontology primary care. She earned her medical degree from University of California, Los Angeles. Prior to joining InnovAge, Teresa worked as a medical-surgical nurse, a geriatric nurse and a nurse practitioner in a variety of medical centers. Her passion is working with the senior population.

Teresa was born and raised in southern California and moved to Colorado in 2015. She is married and has several pets including cats, fish, chickens and a dog. She enjoys cooking, hiking and spending time with loved ones.
