  • What is InnovAge PACE?

    InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare and social engagement to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at a PACE center in their community. Each person in the program has a team of medical experts and a personal care plan that is focused on providing a personalized PACE experience. InnovAge is one of more than 100 PACE providers across the U.S.

  • Who can join InnovAge PACE?

    To participate in PACE, you must be

    • At least 55 years old;
    • Eligible for nursing facility level of care;
    • Able to live safely at home and in the community with support; and
    • Living in an InnovAge PACE service area.

    To qualify for PACE, seniors must meet certain financial and medical requirements. Please note InnovAge offers PACE in multiple states, and each state has its own requirements to qualify. For more details, and to talk with one of our senior care experts, call us at 844-704-9613 TTY:711.

    If you need more information on how PACE works with Medicare or Medicaid, please call us at 844-704-9613 TTY:711.

  • Does anyone live at an InnovAge PACE center?

    No. The goal of PACE is to keep older adults living safely at home. Our PACE centers are safe, friendly and stimulating places. Care teams and participants decide how often the participant attends the PACE center for medical needs and social engagement. Participants return to their own homes each night.

  • Can I join without going to a PACE center?

    Your attendance at the PACE center is based on your medical and social needs. One of the benefits of PACE is the personalized care each participant receives. Some participants only visit the center for medical appointments. Others come more often to socialize, eat meals and enjoy activities.

  • Can I keep my regular doctor?

    PACE participants see a doctor and have access to a full medical team at their local PACE center. The doctors and many members of the medical team are experts in senior healthcare. All of them are dedicated to giving personalized care to each participant. Once particpants are enrolled, many find they have more quality time with their PACE doctor than they had with their previous doctor.

    Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services), from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.

  • How much does it cost?

    Most of our participants enjoy PACE at low or no cost:

    If you are: Cost (out-of-pocket):
    Eligible for Medicaid No cost*
    Eligible for Medicare and Medicaid No cost*
    Eligible for Medicare only Pay Medicaid portion, plus Medicare Part D
    Not eligible for Medicaid or Medicare Self-pay rate

    *If you live in or move to supportive housing (assisted living or nursing home), there is a co-pay to that facility. Also, some states require a share of cost fee if your income exceeds the Medicaid threshold.

    Some participants choose to pay privately for PACE, though they still need to meet all eligibility requirements for their state.

    To find out more about costs, call an InnovAge senior care expert at 844-704-9613 TTY:711.

  • Where do I see my primary care doctor?

    PACE participants see their primary care doctor at the PACE center.

  • I really like my specialist (cardiologist, oncologist, etc.). Can I keep seeing him/her after I join PACE?

    InnovAge has a network of specialists in each community. If your specialist is already in the InnovAge network, and the care is coordinated through PACE, you may be able to see the same specialist. If the care is approved by your medical team but your specialist is not in the InnovAge network, InnovAge will coordinate your care with an in-network specialist.

    Our PACE clinics arrange all specialty medical appointments and transportation for participants. If a family member or friend is unable to go with you, a member of your care team can accompany you.

    Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge.
